We have taken a huge number of steps to ensure anonymity in this program. We feel that you will be able to supply honest answers without
intimidation or fear of retaliation if your identity is private. This survey has been tested over and over again to ensure its security.
When you are ready to take this survey, you will first log on by supplying a "secret word".
This will give you a 4-digit code that you will need to begin the survey.
When prompted, type in your 4-digit code and secret word and you can begin. Your secret word can be a total of 20 alpha-numeric digits
of your own choice. You will need to remember this word in order to complete this survey. And, in case you are unable to complete the survey, you can return at a later time and
log back in by re-supplying the same secret word. You must, however, complete your answers within your employer's alloted run-time of the survey.
We suggest you use something that is easy to remember, like your mother's maiden name, your cat or dog's name, the city you were born in, etc.
With each question, there is a "comments" area. You are highly encouraged to supply a comment with each and every question. This is not required or mandatory.
However, the answers you have given will be more effective if you supply additional information to describe your reasons or basis for your answers. The reviewers
will appreciate additional input, either positive or negative. The comments will be seen by the reviewer(s) but the identity of the person who supplied these comments is still not revealed.
So, be honest and be as detailed as you feel necessary. Do not use comments that can reveal your identity, like "He gets angry with me when I forget to lock the office door when we close for the day."
When you have completed the survey, your answers and comments are locked in to the survey. No one will be able to view them, alter them or determine the person
who supplied these answers. The answers and comments are all compiled in the results at the end of the survey. And the identity of each individual who supplied these answers and comments
will not be known, even to the reviewer(s) who ordered the survey.
