Terms and Conditions of Use for this Program

This program is not intended to be a psychological evaluation tool. This is a performance evaluation program.


360 Custom Surveys is designed for the purpose of evaluating personnel within a corporation or special district entity. It is intended to provide the vehicle to evaluate the productivity or qualifications of an individual or group, as seen by those who work with or directly under the person(s) being evaluated. This service provides only the means of conducting these surveys and not the outcome concluded from any survey.

The questions are the responsibility of those who engage our services and/or produce their own questions for the program they wish to employ.

The answers are the responsibility of those who complete the survey at the instruction of their employers or board of directors.

The results produced from these questions and answers are generated automatically and are not the responsibility of 360 Custom Surveys, its owners, sponsors or personnel. What is done with the information gained from this survey is the sole responsibility of those who ordered the survey.

Custom-tailored programs remain the property of 360 Custom. By purchasing these programs, you are merely granted a license to use this program and are bound to the terms of our (End Users Licensing Agreement) unless a prior written agreement is negotiated between your corporation and 360 Custom Surveys.

By using this program, you agree to adhere to these terms and conditions.

As a Survey Sponsor:
By purchasing this program, you certify that you are authorized to do so by a board of directors, a commanding officer or other senior personnel within your organization. Use of this service by unauthorized persons for the purpose of attacking any individual, public entity or corporation is prohibited. Attempts to create surveys by persons unauthorized by their supervisors, board members, etc. will be considered as fraudulent and the attempt may be reported to the authorities as well as those who may be targeted by such fraudulent attempts.

As a survey taker:
You are expected to answer as many questions as may be pertinent to your position as an employee of a corporation, public entity or private company. Honest answers are strongly suggested, whether they are positive or negative. Use of profanity is prohibited.
Your identity is anonymous, so you can feel free to express your exact feelings about the person(s) or subject of any survey. However, use of foul language is not encouraged and can serve no real purpose other than allowing you to vent your frustrations. Remember, even though you are anonymous, the collective comments are all made known to the person(s) conducting the survey. Keep your answers clean, but express your true feelings to each question. This survey system is designed to improve workplace environments, not create more hostile workplace situations.

360 Custom Surveys is not responsible for data loss due to computer malfunctions, storage disk failures, internet communication problems, weather-related problems or malicious "hack attacks".
